WELCOME TO THE WISH ADOPTION FORM – QUESTIONNAIRE ONLY !!!

Thank you for your interest in adopting a rescue animal. This is ONLY the WISH adoption questionnaire, not an adoption form. The information you provide, helps us to make sure the pet you adopt is the perfect fit for you and your family!

To help make this process as easy as possible. Please answer the questions below, with as much information as possible. Once we have your questionnaire submitted. Your questionnaire will be returned to you – requesting photos of your yard, gates, fence lines and any existing pets.

Adoption Costs:
Adults – $850 (includes GST)
Puppies (under 6 months) – $850 plus $100 sterilisation bond. Adopted on a puppy contract to be sterilised at 6 months. Once sterilised the $100 puppy bond is reimbursed, after the sterilisation certificate has been forwarded to wishrescueteam@gmail.com along with your bank details.

This Adoption fee covers the vet work the animal will receive from WISH, which includes:

Sterilisation- compulsory to contract at 6 months.
Vaccinations – 2 or 3 depending on age.
Microchipping.
Flea and Worm treatments.
A thorough vet check and anything else may of been needed, whilst in WISH FC.

Please note:

This questionnaire may be completed by several people, who are interested in the same pet at the same time. If this happens, you may be sent to one similar to the one on your enquiry form.

Also, sometimes the pet you apply for may not be the perfect fit and we will send you to meet others, that we feel would be better suited.
